Home Visit Forms for Each New Friend
The 2 Page Home Visit Form provides basic demographic information, contact information, sacramental information and special needs information. This form is for your SPRED Community's files and use. These do not go to the SPRED Center. Basic demographic Information from these will be used to complete the Profile Forms.
The Consent for SPRED Participation now called the Permission Form and Waiver Template is to be completed and signed annually by the parents or guardian and allows the Friend to participate with options to consent for transportation if necessary and photo consents that may be used for bulletins, websites. Identifying tags on photos with names are not used for privacy reasons unless there is a separate consent expressly for this purpose.
Profile Forms
Profile Forms are used at the beginning of the year to inform the SPRED Center who is participating both catechists and Friends. This allows for correct billing of the associated parish that the Friend is from and to oversee the number of volunteers participating. The information filed with the SPRED Center by the PC is what appears in the SPRING PLANNING MEETING mailing material..
Retired-Transferring Form is to report changes as the SPRED year begins and can be forward to the SPRED Center at anytime.

Volunteer Driver Form. (Click title to view.) If you routinely or occasionally find it necessary to provide transportation, complete this form with the driver's information and keep with a copy of their auto insurance. The Parish Chairperson has the responsibility of keeping this information on file in the Parish.
